Customer Own Materials (COM) Form
Thank you for participating in Oly's Customer Own Materials (COM) program. If the lead time is pressing, Oly also stocks select frames in muslin. If the install date is not pressing, Oly's COM is a compelling option. The Oly upholstery team is based in Reno and is comprised of seasoned experts in the craft of custom upholstery.
Note by participating in the COM program, the designer agrees to abide by the following COM specific terms:
- All COM orders are subject to standard Oly Terms & Conditions
- Fabrics must be continuous yardage and sent on a single roll
- Fabrics must be at least 54'' wide
- Oly does not accept multiple pieces or folded fabric
- Inspect all fabric. Oly is not responsible for fabric that arrives damaged
- Oly reserves the right to reject fabric for upholstery
- Oly does not guarantee COM for washability
Oly is happy to provide pre-upholstery estimates on yardage. Estimates can/may vary.
Please note: Additional yardage may be required. Oly will not reimburse for any additional yardage required.
The following 2 easy steps MUST be completed to successfully submit COM fabric and form.
COM forms can be submitted by mail, via fax, or scanned & emailed to your inbound sales rep.
Note:
- Do not fill out / submit COM form until you have received a tracking number from the fabric supplier
- Upholstery will not begin until the form and fabric have been received
- Orders are subject to cancellation if fabric and form are not received within 45 days of order
- Clearly specify directions for any trim detail submitted
- If more than one fabric is submitted per piece, clearly specify on the form where each fabric is to be placed
- Designer accepts all responsibility for precise specifications and knowledge of fabric types and repeats
To submit a COM form via mail, fax, or email, download and complete this form in duplicate:
- Send a copy to the fabric supplier per above to attach to the outside of the roll
- Keep a copy for your records
- Send the original with a 3" square swatch to Oly

If not noted in special instructions, upholstery will be applied as OLY standard.
Note: Do not send fabric until you have an Oly Sales Order Number.
When you've chosen the fabric:
- Direct the fabric supplier to attach a copy of a completed COM form to the outside of the fabric roll
- Make sure the fabric supplier attaches a completed copy of this mailing label to the outside of the fabric roll
- Unidentified rolls will be refused and returned
If bolt is turned on end and fabric rolls out from left to right, the pattern is continuous across the roll.
If bolt is turned on end and fabric rolls out from top to bottom, the pattern is continuous down the roll.
Measuring Fabric / Special Instructions
- It is the designer’s responsibility to determine the appropriate amount of fabric needed based on positioning and matching
- Measure carefully – inadequate yardage will delay your order
- If uncertain, always add additional yardage
- Orders without explicit instructions or sketches will be optimized
- Please make sure COM forms are submitted at one time
- If using plain 54" fabric, please refer to the Oly Price List for your yardage requirements
